This procedure is to be followed when an employee require to work from home (ie: ## access to office computer via home computer, company laptop or login to AVD/VDI account):


1. To seek HOD permission to let you work from home 

2. To raise Telecommuting Agreement Form (refer attachment)

3. Seek HOD endorsement  on the form.

4. Send form to respective location HR Dept (for AVP, Manager for Approval)

5. Once HR approved, to send via email to AVP, ITSM, attaching the duly approved Telecommuting form. . 


Following information is required for IT dept to do WFH provisioning :

   i. Fill up the employee details (eg. Username) 

   ii. Provide you office equipment (eg: Office PC Name of your company  PC/Laptop or to indicates that your using VDI/AVD account)



## : To note that separate forms are required to request Access to Email via outlook web access, or via Mobile Outlook Application.
